I could get my head round addressing someone as Madam Captain/President if I had to, its just Lady Captain/President that stumps me

Its not actually a title other than Lord/Lady, which isn't relevant in this scenario

I say it in a friendly way with a almost jokey emphasis on the sir!

...well from 10.0 to 9.4 this year would be nice.

And with just the October medal to go - I've got to 8.3

Indeed - but for me being a member of a golf club is more than just having somewhere to play golf.

Hopefully in the style of someone from Allo Allo. perhaps the Italian officer . A click of the heels would be a nice touch as well.

We address ours a Mr Captain as he is a man.
We call the ladies captain Margaret as she is only Captain for the ladies.
We have never had a woman captain so I assume it would be Madam Captain and call the men's Captain By his name!
    So you have 3 captains?

We also have 3, Mens, Ladies, Seniors (mens)

I don't know if any one captain has seniority, never thought about that before. I assume not, all are equal as far as I am aware.
